Agency Referral Application

Bright Futures provides a range of support services to young women, including one-to-one help, group sessions, community programmes and health and wellbeing clinics that directly address the key issues they face today.

It is crucial that you provide Bright Futures with ALL the information in the Agency Referral application form by completing as many fields as possible, so that the service can make an informed judgement about the child’s needs and act accordingly.

Without a good quality referral, Bright Futures is unable to make appropriate and proportionate decisions. This can put a child or young person at risk or lead to overly intrusive interventions which are disruptive to the child and/or family.
